    Those that possess detoxicating action and effect of removing swelling and can promote the evacuation of pus and regeneration of tissues for the treatment of carbuncle, cellulites, sores and furuncles are known as prescriptions for treating carbuncles.
     
    Carbuncles, cellulites, sores and furuncles may appear on the surface of the body (briefed as external carbuncle) or occur inside the body (briefed as internal carbuncle). Therefore this group of prescriptions is classified accordingly into two categories, namely the prescriptions for external carbuncles and those for internal carbuncles.
     
    External carbuncles with local redness, swelling, pyrexia and pain, are known as yang syndrome and those with local diffuse swelling, hardness or softness, unchanged skin color in a scattered way are known as yin syndrome. Treatment of external carbuncles comprises external and internal therapies. Discussed in this chapter are only prescriptions of the latter therapy. In the application of internal therapy, resolving method, expulsion method and tonic method should be employed respectively in the light of the three stages of carbuncle development (i. e. initial stage, pus formation and post ulceration stage). Resolving method is used at the initial stage of non-suppuration to eliminate toxins and subdue swelling.
     
    In case of yang syndrome, prescriptions should be aimed at clearing away heat and toxins, while in case of yin syndrome, aimed at dispelling cold and removing phlegm. Expulsion method is employed at the intermediate stage of carbuncles when pus is formed but difficult to be expelled due to deficiency of vital-qi and retaining of pathogenic factors. This method can cause the internal toxins to shift to the superficies of the body, and make pus easy to fester and the wound to heal soon. Tonic method is often applied at the later stage of carbuncles manifested as deficiency of both qi and blood, thin clear watery pus and unhealed vent. The tonic drugs can replenish qi and promote the production of blood and tissue regeneration.
     
    Prescriptions for internal carbuncles mainly consist of drugs with the function of clearing away heat and toxins,removing stasis and draining pus, which can clear away heat from the vescera, remove stasis and evacuate pus as well as resolve mass and promote wound healing.
     
    It is the first priority to differentiate the location,nature and different stages of carbuncles in the application of these prescriptions so that corresponding prescriptions could be employed properly. It is necessary to adopt tonic method at the later stage of carbuncles with deficiency of vital-qi, but not too early or exclusively so as to prevent the unremoved toxins from lingering on.
     
    Prescriptions for External Carbuncles
     
    Xianfang Huoming Yin
    ( Fairy Decoction for Treating Cutaneous Infections)

    Ingredients :
    No. 1 Baizhi (Radix Angelicae Dahuricae ) 3 g;
    No. 2 Beimu (Bulbus Fritillariae) 3 g;
    No. 3 Fangfeng (Radix Ledebouriellae) 3 g;
    No. 4 Chishaoyao ( Radix Paeoniae Rubra) 3 g
    No. 5 Dangguiwei ( tail of RadixAngelicae Sinensis)3g;
    No. 6 Gancao (Radix Glycyrrhizae) 3 g;
    No. 7 Chaozaojiaoci (Spina Gleditsiae) (parched) 3 g;0.8 Zhichuanshanjia (Squama Manitis Praeparatae)3g;
    No. 9 Tianhuafen (Radix Trichosanthis) 3 g;
    No. 10 Ruxiang (Olibanum) 3 g;
    No. I 1 Moyao (Myrrha) 3 g;
    No. 12 Jinyinhua (Flos Lonicerae) 9 g;
    No. 13 Jupi ( Pericarpium Citri Reticulatae ) 9 g.
     
    Administration : Decoct the above drugs in water or in the mixture of water and wine with equal amount for oral application. The residue can be smashed for external application.
     
    Actions: Clearing away heat and toxins, subduing swelling and disintegrating mass, promoting the circulation of blood and relieving pain.
     
    Clinical Application: This recipe is indicated for carbuncles in the early stage, marked by local redness,swelling, pyrexia and pain, pus being not yet formed or formed already, or feverish body with slight aversion to cold, thin whitish or thin yellowish fur, rapid and forceful pulse. It is applicable to such diseases with purulent inflammations as phlegmon, suppurative tonsillitis, mastadenitis, impetigo herpetifomis, furuncle, deep abscess, and others, which pertain to yang syndrome and excess syndrome. In case of severe heat-toxin and local redness and swelling, add Pugongying (Herba Taraxaci), Lianqiao (Fructus Forsythiae ) , Zihuadiding ( Herba Violae ) and Yejuhua (Flos Chrysanthemi Indici ) to strengthen
    the effect of clearing away heat and removing toxins. In case of constipation, add Dahuang (Radix et Rhizoma Rhei) to purge away fire and relax bowels as well as to lead heat downward. In addition, guiding herbs could be added in accordance with the location of carbuncles so as to cause the drugs to take effect directly at the affected part.
     
    Elucidation: The syndrome is caused by excessive accumulation of heat and toxins inside, leading to qi stagnation, blood stasis and stagnation of phlegm. This should be treated by clearing away heat and toxins, subduing swelling and resolving mass, promoting blood circulation and relieving pain. Because of the effect of eliminating heat and toxins as well as subduing swelling, No. 12 was thus named holy herb for carbuncles by our forefathers and functions as monarch drug in this recipe. No. 5, No. 4, No. 10, No. 11 and No. 13 have the function of promoting the flow of qi, removing obstruction from the meridians, promoting blood circulation and resolving stasis so as to subdue swelling, all playing the role of minister drugs. No. 1 and No. 3 can expel wind and heat, No. 8 and No. 7 can ensure smooth flow of qi in the meridians and collaterals, promote pus discharge and resolve mass. No.9 and No. 2 possess the function of clearing away heat, removing phlegm, resolving mass and subduing swelling so that pus is resolved before it can be formed. These six ingredients act as adjuvant drugs together. No. 6 is the guiding drug with the action of clearing away heat and toxins as well as mediating drug properties. The wine is meant to enhance the potency of the recipe so that all ingredients will quickly take effect at the affected part because wine can ensure smooth circulation of blood in the vessels.
